 Travelers' Health and Immunization Services has closed. The owner, Dr. Leonard Marcus retired effective December 31st, 2007. Clinics and physicians providing pretravel advice and immunizations can be found on the web sites of the International Society of Travel Medicine, www.istm.org and the American Sociey of Tropical Medicine and Hygiene, www.astmh.org. Physicians who passed the certifying exam in tropical medicine and travelers' health are indicated with * in the clinical directory at www.astmh.org and are uniquely trained to diagnose and treat infectious and parasitic diseases, including those associated with travel or transmitted by ticks, insects and animals. Former patients of Travelers' Health and Immunization Services who want copies of their records sent to themselves or another physician must download and fill out this Form. Please sign it and mail it to Dr. Leonard C. Marcus, 1555 Commonwealth Av., Newton MA, 02465. Enclose a stamped self-addressed envelope. There is a $15 service charge for each record requested, payable by personal check.
